穴位效应是针灸临床疗效的关键,其中相关化学物质的应答变化是穴位效应的一个重要环节。刮痧、针刺、艾灸在临床中广泛应用,常常对于同一种病症均具有较好的治疗效果,然而这些不同刺激方法发生效应的共有性和差异性缺乏系统的研究。本研究围绕“穴区对不同刺激方法的表达具有共性和特性,是刮痧、针刺、艾灸产生相应各具特点效应的基础”的假说,采用荧光免疫组织化学技术和分子生物学技术,定性定量比较刮痧、针刺、艾灸不同刺激对穴区局部表达的影响;并选择刮痧、针刺、艾灸疗效较好的坐骨神经痛为研究载体,观察刮痧、针刺、艾灸穴区局部组织中化学物质的应答变化,以对坐骨神经痛的效应机制为切入,分析穴区不同变化引起的局部和远端效应,阐释刮痧、针刺、艾灸不同刺激方法起效的基本要素,以期为复杂的穴位效应和不同针灸方法的临床准确使用提供一定的科学依据。

The effect of acupoint is believed to be a key factor in the clinical efficacy of acupuncture, and the response change of chemical substances is an essential link in the effect of acupoint. Guasha, acupuncture and moxibustion are widely applied in the clinical treatment, and produce satisfied treatment outcomes faced with the same disease. However, the common mechanism and differences for the effect of these different acupuncture stimulation methods are in need of systematic study. Due to the superior efficacy by Guasha, acupuncture and moxibustion, the model of sciatica is selected as research carrier, and the response changes of chemical substance by Guasha, acupuncture and moxibustion in acupoint area are observed; with the effect mechanism of sciatica, the local and end effects in acupoint are analyzed. As a result, the essential factors for the effect of Guasha, acupuncture and moxibustion is explained, which could provide certain scientific basis for the complicated acupoint effect and the clinical application of different acupuncture stimulation methods.
