胚胎质量的形态学评分的主观性使得临床采取双胚胎移植策略,从而引起多胎妊娠的风险及并发症。因此,急需一种全新的评估胚胎质量的方法,与形态学评估联合使用以提高临床妊娠率及活产率。布鲁塞尔自由大学卵泡生物实验室的回顾性研究的数据已支持有确定的7个卵丘细胞基因与ICSI胚胎质量及妊娠结局强相关,且已建立有效的预测ICSI胚胎发育潜能的模型,并在临床得到验证:可以将D3单胚胎移植的妊娠率从30%提高到60%。为了准确评估IVF卵子质量及胚胎发育潜能,需要重新探索颗粒细胞的基因表达,建立全新的IVF 妊娠结局的预测模型。然而,与欧洲国家不同的是,国内生殖中心对是否行 ICSI操作有着严格的临床指征,从而国内接受ART治疗的患者中仅有20%-30%的为ICSI周期。中山大学附属第六医院生殖医学中心全年完成5000个周期以上,具有稳定的临床妊娠率和活产率,居中国前列。因此,以共同的研究兴趣为前提,我中心与布鲁塞尔自由大学强强联合,技术和临床实力优势互补,共同合作推广这种全新的、非入侵性胚胎评估的方法,率先建立预测IVF胚胎质量的模型,为更安全、高效地提高单胚胎移植的临床妊娠率做好示范作用。

英文摘要

Nowadays in the field of assisted reproductive technology (ART), selection of good-quality embryo for transfer is mostly based on the morphology of embryos. In order to obtain acceptable clinical pregnancy rates for our patients, double embryo transfer is applied, which bears a risk of multiple pregnancy. Therefore, an innovative method for evaluating the quality of embryos is urgently needed, which may improve the clinical pregnancy rate and live birth rate to be combined with morphological evaluation in an algorithm. The Follicle Biology Laboratory (FOBI) of the Free University of Brussels (VUB) had performed over the last 2 years proving that combining the CC gene expression with morphology greatly improved the choice between "equally good looking embryos" and dramatically increased the pregnancy rate in ICSI patients when a single embryo D3 was transferred on D3 (at 4-8 cell stage). However, the gene expression in CC of oocyte undergoing IVF may differ from that of ICSI where sperm is directly injected into the oocyte. While the majority (70%) of couples at VUB are treated by ICSI, the majority of the patients (80%) in our Center (6th Hospital treating 5000 cycles) are treated with classical IVF. From the premise of the common research interests, we and the University clinic of the VUB decided to integrate technical and clinical resources to obtain complementary advantages. Hence, research teams from Guangzhou and Brussels decided to integrate technical and clinical resources to improve research speed. The ambition of our collaboration is to build a new algorithm capable to predict the developmental potential of IVF embryos using a non-invasive method. This capability would increase the utilization rate of single embryo transfer in IVF (still most often used in China) by increasing the prediction of the developmental competences in the oocyte cohort that is fertilized by IVF. The far aim of this collaboration is to bring together competences from two large and experienced ART groups in order to reduce multiple pregnancy rate and its costly complications and to reduce time-to-pregnancy in Chinese subjects, by introducing an innovative and non-invasive molecular analysis on cumulus cells.
