失眠证已经成为危害人类健康的全球性多发病。针刺治疗原发性失眠疗效确切,尚缺少效应中枢特异性方面的研究。平衡针是国家推广的针灸适宜技术,是以源于中医心神调控的“中枢调控学说”为理论核心,认为治疗定位在脑,但目前平衡针治疗原发性失眠的中枢脑靶轴尚不明确。本课题选择临床原发性失眠患者为研究对象,采用平衡针"外周神经靶点"失眠穴,进行干预;联合应用静息态功能磁共振成像(rs-fMRI)和磁共振弥张量成像(DTI)技术,以LACC网络连接为切入点;对原发性失眠患者进行干预前、干预后的脑网络活动影像化,与安慰针对照组和健康对照组进行图像对比,并与临床疗效指标进行相关性分析。从功能连接和结构连接两个维度,阐明平衡针治疗原发性失眠的脑靶轴及网络连接特征,证明原发性失眠针刺效应中枢的特异性,揭示“中枢调控学说”的科学内涵,为平衡针理论和临床应用提供实证依据。

Insomnia is a common disease globally. Acupuncture is effective in primary insomnia. However, how brain is involved remains unclear. Balance acupuncture is a Chinese medicine therapy commonly recommended, and its action is considered to be mainly associated with the central regulation. However, it is unclear where the specific target of the brain axis is.This study includes participants with primary insomnia, who are allocated to the intervention group, which is treated with"insomnia acupoint"of balance acupuncture, placebo acupuncture and compared with healthy participants . Multi-modal functional MRI (rs-fMRI, DTI)is used to compare the dynamic brain images of the participants before and after the treatments, in order to elucidate the target of the brain axis and its connection characteristics, from the perspective of functional and structural connections of the LACC’s network. This study aims to provide the evidence to support the clinical application of balance acupuncture for primary insomnia.
