针刺治疗帕金森病(Parkinson's disease,PD)能有效改善患者的临床症状、减少药物副作用,但中枢机理未明。我们前期研究发现不同亚型(震颤为主型、非震颤为主型)PD患者具有不同的神经基础。但既往针刺治疗PD中枢机制研究并未选取不同亚型PD患者进行针对性研究,而且观察指标多集中于脑局部活动异常或不同脑区间的连接异常,未能反映全脑整体功能的变化。我们推测震颤型PD患者存在特定的局部/全脑功能异常,针刺治疗有可能通过调整异常的脑神经网络,使之重组或增强某些脑区代偿功能,使之趋向正常脑功能状态转化。为验证这个假说,本实验采集震颤型PD患者针刺治疗前后的功能磁共振数据,构建全脑功能网络,采用基于图论的分析技术研究网络拓扑属性,分析震颤型PD患者针刺治疗后的网络属性变化,并与临床症状评分进行相关分析,探讨针刺治疗PD中枢效应,为将来临床针刺治疗提供基础理论支持。

Acupuncture treatment can effectively improve the clinical symptoms of patients with Parkinson's and reduce the drug side effects, but the underlying central effect is largely unknown.Our recent studies indicate that it is different of the neurological basis of tremor-dominant and non-tremor subtype of PD.But previous studies don’t focus on the subtype of PD patients, and mainly explore acupuncture treatment from the perspective of local regional metabolism or specific functional connectivity, leaving whole-brain network changes unstudied.Here, we presume that tremor-dominant Parkinson's disease alters the brain network topology and acupuncture treatment can regulate and further adjust these abnormalities to return to or close to the normal state.To test this hypothesis, we collected resting-state functional magnetic resonance imaging data before and after the acupuncture treatment of patients with tremor-dominant Parkinson's disease, constructed interregional functional connectivity networks, and then utilized graph-based network approaches to study the topological properties of these networks. Furthermore, we compared these network metrics before and after the acupuncture treatment of Parkinson's patients to explore acupuncture central effect and provide a basis for theoretical support for clinical acupuncture treatment in future.
