中文摘要

微波消融突破了传导加热模式热效率较低的局限。以超声引导微波消融治疗肿瘤为主线,申报人在阐明微波消融热场优势规律和血流热沉降对微波热场干预机制基础上,深入揭示了微波消融肝癌后复发转移生物学规律及表观遗传机制,明确了ZNF545为肝癌抑癌基因且其甲基化是肝癌消融预后风险因素,为建立肝癌消融联合表观遗传治疗的个体化新模式提供依据,同时拓展建立了微波消融肾、脾肿瘤的技术体系并明确循证医学依据。以第一作者发表SCI论文11篇,最高IF 14.9,2篇发表于影像专业权威杂志Radiology (IF 6.3)并获专文书面述评。获2011-13年国内放射界最具影响力前10名SCI文章第一名。受邀Springer主、参编微波消融肝癌专著各1部。本课题拟通过构建阳离子碳酸氢铵脂质体载阿霉素纳米颗粒联合微波消融肝癌,解决单一消融难以实现三维空间复杂肝癌彻底灭活难题,为提升肝癌精准消融疗效提供新思路。

英文摘要

Microwave ablation (MWA) breaks through the limitation of low thermal efficiency of conduction heating mode. Focusing on ultrasound guided precision MWA of tumor, on the basis of revealing the law of MWA thermal field advantage and the sink effect of blood flow on MWA thermal field, the applicant revealed biological and epigenetic mechanisms of liver cancer recurrence after MWA,and clarified the methylation of ZNF545 gene as tumor suppressor gene was a risk factor for recurrence after MWA of liver cancer, which laid important basis for ablation combined with epigenetic therapy for liver cancer. Meanwhile, established the technical systems of MWA of kidney and spleen tumor and clarified the foundation on evidence-based medicine. The applicant published 11 SCI included papers as the first author with the highest IF 14.9, and 2 articles published in flagship magazine of imaging specialty-Radiology (IF 6.3) with the positive review. Obtained the first of top 10 most influential SCI articles in the domestic imaging field during 2011-13. Be invited by Springer compiled 2 monographs on MWA of liver cancer as editor-in-chief and editor, respectively. This research proposes to build a nanoparticle of cationic temperature sensitivity liposome loaded adriamycin, which will be used by combining with MWA. The combined therapy is expected to solve the difficulty that single ablation is difficult in realizing completely necrosis of complex liver cancer in three dimensional space. The research will provide a new strategy for improving precision effect of MWA in liver cancer.
