太极拳姿势训练应用于脑卒中偏瘫步态康复具有良好的理论基础,其临床疗效也得到了初步验证。本研究拟应用RSSCAN步态系统对其临床疗效的生物力学机制进行探讨。研究拟纳入144例符合标准的脑卒中偏瘫患者进行RSSCAN步态系统测试,并以匹配的健康受试者为对照,探讨脑卒中偏瘫患者各生物力学指标的特异性改变以及与临床量表的相关性。然后将144例患者随机分为对照组和试验组,对照组接受现代康复治疗,试验组在对照组基础上加入太极拳姿势训练。两组均进行4周康复治疗和3个月随访。分别在入组时、治疗及随访结束时进行RSSCAN步态系统测试和临床量表评价,对比分析两组患者在生物力学指标和临床量表的改善以及二者的相关性。借以解读太极拳姿势训练促进脑卒中偏瘫患者步态康复的生物力学机制,并进一步回馈和完善其理论基础,为在脑卒中偏瘫患者的康复过程中应用和推广太极拳康复训练进行前期铺垫。

There is good theoretical basis in applying Tai Chi posture practicing for stroke hemiplegia patients and the clinical effects have been preliminary confirmed. We tried to study the biomechanical mechanisms of Tai Chi for stroke hemiplegia using the RSSCAN gait system in the current study. A total of 144 stroke hemiplegia patients and 144 matched healthy subjects will be recruited to undergo the RSSCAN gait system test. Differences of biomechanical outcome measures between stroke patients and healthy subjects will be studied with clinical scales. Then the stroke patients will be randomly assigned into a study group and a control group. Patients in the control group will receive conventional rehabilitation techniques while patients in the study group will receive Tai Chi posture practicing plus conventional rehabilitation techniques. The rehabilitation treating course will last 4 weeks with 3 months following up. All patients will be tested with the RSSCAN gait system and clinical scales at baseline, 4 weeks after treatment course and 3 months after the follow up. We will compare the results within and between two groups to study the effects of rehabilitation techniques and the biomechanical mechanisms of Tai Chi posture practicing. The results of the current study will provide objective evidence to detect the biomechanical mechanisms of Tai Chi for stroke hemiplegia rehabilitation and may contribute for the further application and promotion of Tai Chi.
