脊髓缺血再灌注损伤(spinal cord ischemic reperfusion injury,SCII) 是脊柱骨折、脊髓供血动脉疾患等手术术后截瘫的最主要的原因,而围术期的 SCII 具有可预见性,因此预处理手段防治SCII ,不仅可行而且意义重大。电针是预处理手段之一,但其作用途径及机制尚不明确,我们前期研究发现TNF、IL-1、IL-6与电针防治SCII有关。TLR4/NF-κB是其上游的炎症信号通路,据此提出TLR4/NF-κB信号通路是电针预处理干预SCII的重要途径之一的假设,我们拟采用电针“悬钟”与“阳陵泉”两穴防治SCII,观察电生理、免疫分子、病理等指标进行研究,来探索TLR4/NF-κB信号通路及下游各环节的物质含量或表达。对阐明电针预处理干预SCII机制有重要意义,为电针预处理SCII更好应用于临床奠定基础,提供从神经-免疫角度研究电针作用机制的新思路。

Spinal cord ischemia reperfusion injury (spinal cord ischemic reperfusion injury, SCII) is the main reason to cause spinal fractures, spinal cord artery disorders postoperative paraplegia and perioperative SCII predictable, so pretreatment method is feasible and significant to prevent SCII. Electroacupuncture pretreatment is one of pretreatment methods but the function channel and the mechanism are not clear. During our previous study we found that TNF, IL-1, IL-6 are associated with acupuncture treatment and prevention of SCII. TLR4/NF- B is the inflammatory signal channel in its advanced position, based on that, we proposed an assumption that TLR4/NF- B signaling channel is one of the important ways that electroacupuncture preprocess to intervene SCII. We proposed two points, ”Xuanzhong" and "Yanglingquan" to control SCII, and studied physiological, pathological observation of immune molecules, electric and other indicators, to explore TLR4/NF- in B signal channels and the matter contents and expression of lower positions, which is significant to illuminate the the effect of electroacupuncture preprocessing to intervene SCII, making a foundation of better use of electroacupuncture preprocessing in clinic,providing new ideas for studying electroacupuncture mechanism from the perspective of neuro and immune.
