阻塞性睡眠呼吸暂停综合症(OSAS)和2型糖尿病(T2DM)是危害国民健康的两大疾病,而二者之间关系又十分密切。为明确OSAS对2型糖尿病患者血糖的影响,本研究拟以动态血糖监测技术(CGMS)对照观察年龄、体块指数严格匹配的正常人、单纯OSAS患者;已进行严格药物治疗的2型糖尿病患者和合并OSAS的2型糖尿病患者夜间血糖的总体水平、血糖波动度的不同,以评估OSAS对血糖的影响;同时对比分析OSAS和合并2型糖尿病的OSAS患者在短期无创正压通气治疗(CPAP)后血糖及胰岛素敏感性的变化,探讨OSAS与糖-胰岛素代谢紊乱的关系。为排除睡眠紊乱的影响,拟进一步进行动物研究,建立不同体型大鼠间歇低氧的动物模型,进行高胰岛素正常血糖钳夹实验,以期从整体水平精确评估低氧血症对机体胰岛素抵抗程度的影响,初步探讨间歇低氧这一OSAS重要的病理生理过程对糖代谢的影响,为OSAS对糖-胰岛素代谢影响的机制进行初步研究

Both obstructive sleep apnea syndrome (OSAS) and type 2 diabetes mellitus (T2DM) are major health hazards for people, and their relationship is very close. In order to uncover the effect of OSAS on T2DM, we intended to observe the night-time glucose level and glucose variability using continuous glucose monitoring system (CGMS) in different groups of age, body mass index (BMI) strictly matched (the normal group, simple OSAS group, type 2 diabetes group, OSAS with type 2 diabetes group).In the same time,we will compare the difference of glucose level and insulin sensitivity after short-time continuous positive airway pressure (CPAP) therapy between the OSAS group and OSAS with diabetes group. Further, in order to eliminate the effects of sleep disorder,we will build different size rat animal model of intermittent hypoxia-the important pathophysiological process of OSAS,and using high insulin normal blood glucose clamps experiment to evaluate the impact of intermittent hypoxia for insulin resistant.
