健康危害严重的冠心病和缺血性脑卒中是由环境因素、遗传因素及环境与遗传交互作用引起的复杂性疾病,但环境、遗传因素及交互作用在这两种疾病中的作用仍不清楚,特别是他们在我国的心脑血管疾病发生风险预测中的作用。本课题以中国慢性病前瞻性研究(CKB)50余万人的超大型队列,开展前瞻性队列设计的病因学研究。采用前瞻性的队列研究重点阐明环境因素(生活方式如吸烟、饮酒、体力活动、饮食等和心理因素、室内空气污染等)、遗传因素(国内外研究发现的的和本项目第二课题发现的新的潜在的易感基因多态性位点)及环境-遗传交互作用对我国人群中冠心病和缺血性脑卒中发生的作用。同时,在此研究基础上,纳入新发现的与传统的环境和遗传危险因素及环境-遗传交互作用,采用Cox比例风险模型构建适合中国人群的冠心病和缺血性脑卒中的疾病风险预测模型,为冠心病和缺血性脑卒中易感人群的筛检和干预措施的制订提供科学依据。

Life-threatening coronary heart disease (CHD) and ischemic stroke are complex human diseases caused by the environmental factors, genetic factors and their interactions. However, till now it is unclear how the interactions of environmental and genetic factors influenced the development of CHD and ischemic stroke. Based on the traditional risk factors for CHD and ischemic stroke, some studies have developed the risk predictive model of cardiovascular disease. However, most of the models were derived from the populations of Western developed countries and had limited predictive ability for CHD and ischemic stroke in Chinese. In addition, because the differences in dietary habit, lifestyle, and genetic structure between Western and Chinese populations, it is necessary to construct cardiovascular disease risk predictive model for the Chinese population. Based on an established mega cohort of China Kadoorie Biobank (CKB) with more than 0.5 million subjects, the present project will explore the etiology of the CHD and ischemic stroke (genetic risk factors, traditional and potential environmental risk factors) and focus on the effects of environment-gene interactions on CHD and ischemic stroke. In addition, the present study will use the Cox proportional hazard model to construct the predictive models for cardiovascular disease that are suitable for Chinese population. Our research will provide scientific evidences for further discovering of novel intervention targets and for risk stratification and prediction of CHD and ischemic stroke.
