采用全身化疗食管癌,疗效差且毒副作用大;金属支架姑息治疗仅能暂时缓解症状,不能降解或溶蚀且易于移位丧失功用。为实现食管癌标本兼治、高效低毒的精准治疗,本项目研究一种新型多功能药物控释高分子食道支架及其介导化疗药物的肿瘤定位、定向、可控释放,实现食管癌局部化疗、磁热疗精准治疗。采用3D打印或自编织技术制备聚酯支架,表面包覆防止药物向腔道内释放的背衬层和负载磁性纳米粒与抗肿瘤药物(如紫杉醇)的干凝胶层。将支架植入食管恶性狭窄部位,干凝胶层吸水溶胀形成黏膜粘附性水凝胶,使支架粘附在植入部位实现局部化、热疗。考察该支架磁热控释药物行为,建立兔食管癌原位模型并评价支架体内释药行为、药物生物分布及抗肿瘤作用,探讨磁热疗与化疗协同增效机制及影响因素,研究支架体内示踪性、粘膜粘附性和抗移位作用、生物可降解性和在发挥治疗作用后自行从消化道消除而不需手术取出的功用。为研发新一代标本兼治抗肿瘤支架提供科学依据。

chemotherapy of esophagus cancer via systematic administration yields poor therapeutic effects and huge toxic and side effects. Metal stent implantation can only relieve symptoms temporarily, metal stent is nondegradable and easy to migrate. To implement precision therapy of esophagus cancer with high efficacy and low tocixity that can cure the symptom and pathogeny, we study a new multifunctional drug controlled release polymer esophagus stent for local chemotherapy and local magnetic hyperthermia combined precision therapy of cancer with tumor-sited and -oriented drug controlled release. Polyester stents are fabricated by 3D printing or braiding, followed by coating a bilayered film of backing layer and dry gel layer loaded with magnetic nanoparticles and chemotherapeutic agent paclitaxel . The stent is placed in the malignant strictured esophagus stie, the dry gel absorbs water to form a mucoadhesive hydrogel, which keeps the stent adhere to the implantation site. We will investigate magnetothermal controlled drug release behaviors of the stent, establish esophagus cancer rabbit model and evaluate in vivo drug release profiles, drug biodistribution and anti-tumour effects; probe the mechanism of synergistic effects of magnetic hyperthermia and chemotherapy and the influence factors; study in vivo imaging,mucoadhesion and anti-movement, biodegradation and self-elimination. The research findings will provide scientific and experiment basis to develop a new-generation multifunctional antitumor stent.
