Dentin hypersensitivity is a common oral short, sharp pain symptom affecting many individuals caused by external stimuli such as cold, hot, sour, sweet and mechanical friction, which has become clinical problems troubling patients and physicians. The occurrence of dentin hypersensitivity was mainly related to the exposure of dentin and the opening of the dentin tubules.Proanthocyanidins(PA), a polyphenols-rich natural extracts, which has been proven to have anti- allergy, anti-inflammatory, analgesic, collagen cross-linking, demineralization inhibiting / mineral deposits promotion and other biological activity, may have some potential to prevent and treat dentin hypersensitivity.Pine bark is the main source of PA, and pinus massoniana is an important pine resource in our country. Therefore, we plan to systematic study the efficacy and mechanism of PA extracted from pinus massoniana bark (Pinus massoniana bark extract, PMBE) on the prevention and therapy of dentin hypersensitivity according to the exposed dentin tubules and pain symptom of dentin hypersensitivity through in vitro and animal experiments, based on the anti-inflammatory and analgesic effects. Safe and effective nature drug for the prevention and early therapy of dentin hypersensitivity, and this will provides reference and scientific basis for the prevention, non-operative treatment of dentin hypersensitivity. New application field of PMBE will also be developed.

牙本质过敏症是指牙齿在受到外界刺激如温度冷热、化学物质酸甜以及机械作用摩擦或咬硬物等所引起的一种异常酸痛症状,是困扰患者和临床医师的难题。牙本质过敏症发生主要与牙本质暴露、牙本质小管呈开放状态有关。原花青素已被证实具有抗过敏、抗炎镇痛,胶原交联、抑制脱矿/促进矿物沉积等多种生物学活性,具备一定防治牙本质过敏的潜质。松树皮是原花青素的主要来源,而马尾松是我国重要的松树资源。本课题通过体外、动物实验统研究了马尾松树皮来源的原花青素(马尾松树皮提取物)防治牙本质过敏症的功效及机制。研究发现:(1)8%和12%PMBE可减弱酸对牙本质矿物密度表面显微硬度的降低,减小牙本质在酸性缓冲液中的钙离子释出率,有效提高根面牙本质耐酸蚀脱矿作用。(2)4%和8%PMBE乙醇溶液可稳定脱矿的牙本质胶原基质,封闭开放的牙本质小管。4%和8%PMBE乙醇组牙本质小管口开放视野面积(ADTO)、牙本质小管口开放相对面积(PADTO)显著小于乙醇对照组,其中8%PMBE溶液可达到与氟保护漆相当的效果。(3)8%PMBE 磨损各组ADTO 及PADTO 均显著小于对照组,牙本质表面可见磨痕及封堵的牙本质小管部分开口。(4)在牙本质磨切预备后1h,1d,3d及7d经8%PMBE溶液处理 的大鼠切牙牙髓炎症反应及P物质、降钙素基因相关肽等神经递质表达均较对照组弱。本研究证实PMBE可抑制牙本质脱矿、封闭开放的牙本质小管口、增强耐刷牙磨损及缓解敏感疼痛,是一种有良好应用前景的防治牙本质过敏的天然物质。本实验可为PMBE防治牙本质过敏提供一定实验依据,还可为进一步研制便于临床局部应用、具备良好缓释效果的抗牙本质过敏的PMBE缓释体系提供一定的实验支撑。
