“证”是中医理论与临床诊疗的核心,证的生物学基础及其演变规律是目前亟待解决的关键科学问题之一。基于课题组既往研究所示代谢组学可反映“证”的整体、动态变化特征,本项目提出“从机体代谢网络的变化揭示湿热证的物质基础,并阐释其相应的生物学内涵”的科学假说。拟以两种疾病(慢性乙型肝炎和非酒精性脂肪性肝病)典型的湿热证患者为研究对象,以相应疾病的无明确证可辨的隐证患者和正常人为对照,运用代谢组学技术检测、分析血液和尿中的代谢物组的变化,结合单维和多维统计分析方法,明确两种疾病湿热证的共同物质基础及差异性代谢物谱。并基于此,以临床常用治疗这两种湿热证的双虎清肝颗粒进行干预研究,基于中医“方证相应”的基本思维,分析在干预作用下代谢物质谱的变化轨迹及其与“证”变化的关联性,解析湿热证发生变化的特征性物质基础和代谢网络,揭示“湿热证”生物学变化的科学内涵,为中医“证”的现代评价与分类提供转化应用的基础。

英文摘要

“Syndrome” is the core of TCM clinical diagnosis and treatment. Whether different diseases with the same syndrome has the common biological basis is a key scientific problems of TCM syndrome theory in modern research. Our previous study shows that the metabonomics can reflect the feature of whole body dynamic change, so in this study,we hypothesize that “the pathological material basis of damp-heat syndrome can be revealed by the the metabolic network”.In this study,typical damp-heat syndrome of chronic Hepatitis B and Nonalcoholic steatohepatitis are studied,non-damp-heat syndrome of above two diseases and healthy subjects are treated as controls, GC/MS(mass spectrometry combined with gas chromatography) and LC/MS(mass spectrometry combined with liquid chromatography) test are used to analyze the metabonomics group changes of blood and urine sample between patients and health persons.Then we will use multi-dimensional statistical analysis techniques to explore the commonness and personality changes of damp-heat syndrome. Further more, SHQGKL, an effective traditional Chinese medicine of damp-heat syndrome, will be used to treated with the patients for damp-heat syndrome,then analyze the metabolites change before and after the intervention, as well as its relation ship to the change of syndromes.So that , we can clear the true value of related metabolites group in judging damp-heat syndrome, build the characteristic metabolites map of the damp-heat syndrome and reveal the main pathological basis of it, and explain the scientific connotation of TCM different diseases with the same syndrome from metaboilic level, and provide the detectability index in judging damp-heat syndrome and evaluating clinical efficacy.
