“梅核气”是一种常见的中医病证,以情绪不舒导致咽部不适感为特征,中医对其的认识历史悠久,实践总结的中药配方行之有效。临床梅核气患者常伴有抑郁焦虑状态,其发病机制尚不清楚。首先考虑梅核气临床特征与现代医学抑郁症躯体感觉异常的咽部不适感非常相似,其次近年脑功能研究不断发现证实抑郁症患者情绪、认知、记忆、感觉相关脑区功能活动和功能连接异常可能与抑郁症及其躯体症状相关。第三,我们在前期研究中,发现抑郁症患者可能存在大脑情绪-躯体感觉异常网络,这可能是其躯体症状存在的原因。故为探究中医“梅核气”的发病机制,本研究欲采用静息态脑功能磁共振成像(rfMRI)和磁共振波谱技术(MRS),对梅核气患者的情绪-躯体感觉异常网络、大脑躯体感觉中枢功能及功能连接和脑代谢进行研究,试探究梅核气是否存在大脑局部功能差异,情绪-躯体感觉异常网络及脑代谢差异的机制,为“梅核气”发病机制及中药治疗提供脑功能和脑代谢学数据。

Globus hysteriocus(GH) is a common disease of TCM , and is characterized by pharyngeal foreign body sensation. TCM has a long understanding history for GH and sums up effective formula by the practice experience. We find that GH patients are often accompanied by depression anxiety state, the pathogenesis is unclear. Considering the GH’s clinical features is similar with modern medicine depression’s pharynx ministry unwell feeling, we intend to refer the explore method of depression with modern medicine to make further study. Recent years, the brain FMRI is widely used in the neuroimaging studies of depression, the study found that depression patients have function and functional connectivity abnormal at brain regions of emotion, cognition, memory and sensory. Then, we also find depressive patients may present emotion - somatic paresthesia network based on our early study, and can be used to explain reason for the existence of the somatic symptoms. In order to explore GH’s pathogenesis, this study uses the resting state brain functional magnetic resonance imaging (rfMRI) and resonance spectroscopy (MRS) technology, to research GH patients’ Emotions - somatic paresthesia network, the functional connectivity and nerve metabolism of pharyngeal sensorium. Try to discuss the mechanism of Brain imaging and nerve metabolism, and provide study data of brain function and brain metabolism for the pathological mechanism research of GH.
