自体皮匮乏仍然是特大面积深度烧伤治疗困难的根本原因。目前,MEEK微型皮片在临床修复大面积烧伤创面得到广泛应用,但对于缺乏皮源的特大面积烧伤,救治成功率会大大降低。表皮细胞体外培养成功使大面积深度烧伤创面的早期修复有了希望,但是单独应用表皮细胞悬液移植修复全层皮肤缺损创面,效果不理想也不稳定。据此,课题将在既往的工作基础上,探讨Meek微型皮片移植、异体表皮细胞联合修复深度烧伤创面的基础研究方案。以最大限度提高自体残存皮肤的利用率,最合理的利用好来源充足的异体皮肤,提高大面积深度烧伤创面修复能力与质量,最终达到提高烧伤救治水平
英文摘要
Early debridement of burns and coverage with autologous skin graft form the mainstay of burn management. However, this is often hindered by an inadequate supply of uninjured skin for grafting. Meek technique for expanding a small piece of skin graft with a dermatomeis widely applied in repair of large area burn wounds, however, lack of autograft donor sites is still an limiting factor in achieving wound closure. The option of autologous cultured keratinocytes may product enough skin to cover the body surface. However, Problems with the lack of “take” and long-term durability, as well as the time delay to produce such grafts, have limited the use of the method. Based on this, This research intends to explore a new think about the use of Meek technique in combination with cultured keratinocytes suspension to repair deep burn wound. This study in order to maximize the utilization of autologous skin and make good use of abundant allograft skin, improves the quality of extensive burn treatment

Meek微型皮片在修复大面积创面中应用广泛,但对于自体皮源极度匮乏的烧伤病人的创面修复治疗,如何进一步提高自体皮的使用效率仍是关键问题。本课题在Meek技术的基础上引入异体表皮细胞的概念,初步建立了细胞-Meek植皮技术。主要内容包括:设计简易Meek微型皮片切割模具,改进了Meek微型皮片植皮技术在大鼠创面修复实验中应用方法;建立表皮细胞重复消化分离法,有效提高了表皮细胞分离提取效率;利用改进的Meek微型皮片制作方法及高效的大鼠表皮细胞分离培养技术,进一步证实应用异体表皮细胞联合自体Meek微型皮片植皮可大幅提高创面愈合率,且通过Y染色体分析发现术后7天可以在创面检测到表皮细胞的存在。本研究通过合理利用来源充足的异体表皮细胞,进一步提高了自体皮肤的利用率,提高了创面愈合效率。
