胰岛移植是目前国际上认为治疗1型糖尿病和终末期2型糖尿病的有效手段之一。在前期研究中,本课题组分别应用成人胰岛细胞和新生猪胰岛对1例糖尿病患者及5例糖尿病恒河猴进行治疗获得成功。然而体外获得的胰岛细胞比较脆弱,对低氧环境耐受性差,易发生凋亡,从而使得胰岛移植远期效果欠佳。本项目在前期研究基础上拟以猪的胰岛细胞为模型,研究PDGF-AA及PDGF-AA结合高压氧预处理对猪胰岛细胞功能及体内外长期存活的影响,并应用分子生物学和形态学技术检测细胞增殖、细胞凋亡及细胞耐受的相关指标,探讨PDGF-AA结合高压氧处理对猪胰岛细胞的保护作用及机制,通过本研究,期望在胰岛细胞体内外存活与保护方面做出有意义的探索,为提高临床胰岛移植的效果探寻新的方法,为糖尿病的防治提供新的实验依据。

Islet transplantation is considered one of the effective treatments for the treatment of type 1 diabetes and end-stage of type 2 diabetes in international community. In our previous study, it was successful that 1 patient with diabetes mellitus and 5 diabetic rhesuses were treated by adult islet cells and neonatal porcine islet respectively. However, islet cells obtained by separation and purification in vitro were more vulnerable, that had the weak endurance to hypoxia environment, susceptible to apoptosis; result in the poor long-term effect of islet transplantation. We plan to pig islet cell as the model of animal studies on the basis of preliminary study, to research on the long-term survival of pig islet cell function by PDGF-AA and PDGF-AA combined with the effects of hyperbaric oxygen preconditioning in vitro and in vivo, to investigate the protective effect and mechanism of PDGF-AA combined with hyperbaric oxygen treatment on pig islet cells, and indexes related to apoptosis and cell proliferation, the tolerance were detected by molecular biology and morphological technology. We want to expect to make a meaningful exploration in the survival and protection of pancreatic islet cells in vitro and in vivo, to explore new method improving the clinical effect of islet transplantation, to provide a new experimental basis for prevention and treatment of diabetes.
