Tongluozhitong external treatment is a treatment formed in the summary of previous treatment of osteoarthritis (OA) experience and years of clinical practice, multi-center clinical trials and animal studies confirmed Tongluozhitong external treatment of OA efficacy exactly, but its mechanism is not yet clear. Preliminary studies found that Tongluo zhitong external treatment can inhibit the expression of inflammatory factors (IL-1β, TNF-α) in the synovial fluid, treat OA by ease the OA synovial inflammation reaction, recent studies have shown that the endogenousdanger signals associated molecular patterns (DAMPs) activate Toll-like receptors (TLRs) to start the NF-κB inflammatory pathways may play an important role in the pathogenesis of OA synovial inflammation. Based on its regulatory mechanism to explore Tongluozhitong external treatment , this subject contemplated by the in vitro and in vivo experiments, research OA development DAMPs / TLRs role of NF-κB inflammatory pathways, clear and definite Tongluozhitong target of treatment of OA,make new explorations on the targeted therapy OA of Traditional Chinese Medicinal.

通络止痛外治法是课题组在总结前人治疗骨关节炎(OA)经验及多年临床实践基础上形成的治疗方法,多中心临床试验和动物实验研究证实通络止痛外治法对OA疗效确切,但其作用机制尚未明确。前期研究发现,通络止痛外治法能够抑制滑液中炎性因子(IL-1β、TNF-α)表达,通过缓解OA滑膜炎症反应起到治疗OA的作用,近期研究表明,内源性危险信号相关分子模式(DAMPs)激活Toll样受体(TLRs)启动NF-κB炎症通路可能在OA滑膜炎症的发病机制中起到重要作用。本课题拟通过体内外实验,通过研究(1)Mankin评分评价大鼠膝OA模型病理分期,并按照Krenn grading of chronic synovitis为滑膜评分。(2)滑膜细胞炎症因子及金属蛋白酶表达:荧光实时定量PCR和酶联免疫吸附法检测各组滑膜细胞IL-1β、TNF-α、MMP-13的转录和表达水平。(3)不同OA病变程度滑膜细胞TLRs表达:荧光实时定量PCR和Western Blot检测各组滑膜细胞TLR2、TLR4的转录和表达水平。(4)不同OA病变程度滑膜细胞及滑液中DAMPs表达:免疫组化法检测各组滑膜细胞及滑液中小分子透明质酸、纤连蛋白、HMGB-1的表达。(5)滑膜细胞中DAMPs/TLRs介导的NF-κB炎症通路重要元件的表达:荧光实时定量PCR和Western Blot检测各组滑膜细胞MyD88、TRAF-6、TRAM、TRIF、NF-κB p65的转录和表达水平,利用荧光染色共定位的方法测定滑膜细胞NF-ΚB 的转位等数据。验证了(1)DAMPs/TLRs介导的NF-κB炎症通路相关滑膜炎症反应在OA发病机制中的作用研究;(2)通络止痛外治法对DAMPs/TLRs介导NF-κB炎症通路相关0A滑膜炎症反应的干预研究;(3)通络止痛外治法君药的主要有效成分对人膝OA滑膜炎症反应及DAMPs/TLRs介导NF-κB炎症通路表达的影响;阐明DAMPs/TLRs介导的NF-κB炎症通路相关滑膜炎症反应在OA发病机制中的作用;证明通络止痛外治法通过调节DAMPs/TLRs介导的NF-κB炎症通路而发挥治疗OA的作用。解决了DAMPs/TLRs介导的NF-κB炎症通路相关滑膜炎症反应如何参与OA发病机制等问题,并为中医外治法靶向治疗OA做出新的探索。
